Roux-en-Y Gastric Bypass
The Roux-en-Y gastric bypass procedure involves creating a small stomach pouch by separating the upper section of the stomach. Subsequently, the small intestine is split, creating a Y shaped digestive tract. This assists to divert stomach acid further down the small bowel, reducing the risk of reflux. This procedure is effective for weight loss as it is both restrictive and malabsorptive.
Why consider a Roux-en-y Gastric Bypass & key considerations
To determine if you are eligible for this procedure, please review the requirements below:
- You have a BMI of 35 or higher
- You have a BMI of 30 - 34.9 with weight related health issues such as high blood pressure or cholesterol, joint or back pain, type 2 diabetes and sleep apnoea.

How is a Roux-en-Y Gastric Bypass Performed?
General Anesthesia
You are placed under general anesthesia so you’re asleep and do not feel any pain during the operation.
Laparoscopic Access
Several small incisions are made in your abdomen.
A laparoscope (a camera) and surgical tools are inserted to perform the procedure with minimal invasiveness.
Creation of the Small Stomach Pouch
The surgeon divides the top portion of your stomach, creating a small pouch about the size of an egg (15–30 mL).
This pouch becomes your new stomach and drastically limits how much you can eat.
Division of the Small Intestine
The small intestine is divided about 45–75 cm (18–30 inches) below the stomach.
The lower portion (called the Roux limb) is brought up and connected to the new stomach pouch. This allows food to bypass most of the stomach and the upper small intestine.
Connecting the Bypassed Intestine
The upper portion of the small intestine (still connected to digestive juices from the original stomach) is then reattached further down the Roux limb.
This allows bile and pancreatic enzymes to mix with the food and aid digestion, just further along the intestinal tract.
Leak Test and Closure
The surgeon tests for leaks at the surgical connections.
All instruments are removed, and the incisions are closed with sutures or staples.

Recovery, Aftercare & Follow Up
Initial Recovery
You will usually spend 1 night in hospital. Dr Puhalla will visit you the next day for a check up and to discharge you home. You are required to have someone pick you up from the hospital.
Pain Management
Pain medication will be prescribed. It is recommended to rest and limit daily activities for 1 – 2 weeks
Hydration
Stay well hydrated, aiming to drink 1.5 to 2L a day taking frequent small sips of water.
Dietary Updates
A 4 staged approach to eating will be guided by the dietitian. This approach starts at liquids, pureed, soft and finally solid foods. This 4 staged approach will be aligned with your dietician appointments post surgery.
Follow Up Appointments
You will have your post operative review appointment 2 weeks after surgery. During the first 6 weeks you will see the dietitian every 2 weeks to guide your dietary upgrades. Regular follow ups with Dr Puhalla at 3 months, 6 months and 12 months to monitor progress and surgical outcome.
Increasing Exercise Gradually
Lifting and exercise restrictions will be applied for the first 10 weeks post operatively.
Incision Care
Dressing can be removed 1 week post operatively. Leave the purple glue in place for 2-3 weeks post op. Watch for any signs of infection such as redness, swelling, heat or discharge. If unsure, call the clinic and speak with our nurse.
Consuming Alcohol
After surgery you will feel the effects of alcohol more quickly than you did previously. It is advised to make a conscious effort to limit your alcohol intake and frequency after surgery. The regular use of alcohol may lead to poor food choices, weight regain, liver damage, reflux and irritation to your gastrointestinal tract.
Cessation of Smoking or Vaping
Your risk of peptic ulcers is significantly increased, therefore we do not suggest you undergo Roux-en-Y gastric bypass surgery if you are a current smoker/vaper. A gastric sleeve is a much safer surgical option if you smoke/vape.

How long does a Roux-en-Y gastric bypass procedure take?
Provided there are no other additional procedures combined, the procedure usually takes 1.5 – 2 hours.
How much weight can I expect to lose?
Weight loss can vary between individuals depending on multiple factors, therefore an exact figure cannot be guaranteed. Most studies show that 2 years after Roux-en-Y gastric bypass, around 79% of excess weight has been lost, and around 70% after 5 years.
How do I know this procedure is right for me ?
- To determine your suitability for this procedure, you will need to have a discussion with Dr Puhalla by booking in an initial consultation. The Roux-en-Y gastric bypass should be my preferred surgical option if I:
- Have severe reflux
- Have metabolic conditions such as diabetes, high blood pressure or high cholesterol
- Eat a diet high in fat or sugar, or tend to binge eat, as negative feedback caused by the bypass will help deter you
- Have a poor weight loss history
- Have a higher BMI
- Have constipation

Is this procedure safe?
The Roux-en-Y gastric bypass has a low complication rate, however every surgery carries risks.
Short term risks can include:
- Bleeding
- Infection
- Blood Clots
- Stomach Leaks
Long terms risks can include:
- Reflux
- Malnutrition
- Hernia
- Vomiting
- Ulcers
- Left sided abdominal discomfort
- Increased frequency of bowel motions

Are there any pre-operative tests prior to surgery?
Yes, in most cases Dr Puhalla will refer you for a pre- operative blood test. He may also refer you for a psychological pre operative assessment to determine surgical readiness.
If clinically indicated, Dr Puhalla may request for you to have a gastroscopy. This is a procedure performed under sedation, where a thin flexible camera is placed down your oesophagus and into your stomach. It can be useful to determine if there is a hiatus hernia, reflux, stomach infections, or ulcers prior to your surgery. A gastroscopy is usually recommended for patients who are over the age of 40, or those who experience reflux symptoms.
Can I still drink alcohol after having a gastric bypass procedure?
Yes you can, however you will feel the effects of alcohol more quickly after bariatric surgery than you previously did. It is advised to make a conscious effort to limit your alcohol intake and frequency. Can I get pregnant after having my procedure?
It is known that weight loss surgery may improve fertility in both females and males. Pregnancy should be avoided in the first 18 months post operatively, due to potential risks to both mother and developing baby. To avoid pregnancy during this time, it is recommended to consider contraceptive options by speaking to your GP. The Oral Contraceptive Pill is less effective after a weight loss surgery, therefore other methods should be used. This is a key consideration if wanting to undergo surgery. If pregnancy occurs earlier than recommended, please contact our team.
Can I have plastic surgery after weight loss surgery?
Yes you can. Some patients may experience loose skin after losing a substantial amount of weight which may affect you physically and emotionally. Speak with our team or your GP to arrange a referral for body-contouring surgery by a plastic surgeon.
I have heard gallstones are common after surgery, will this effect me?
