Loop Duodenal Switch
The Loop-Duodenal Switch (SADI-S / SIPS) is a combination of two procedures. First, a sleeve gastrectomy is performed. A joint is then created just below the sleeve gastrectomy, and a significant portion of the small bowel is bypassed. This procedure is effective for weight loss as it is both restrictive and malabsorptive.

- You have a BMI of 30 - 34.9 with weight related health issues such as high blood pressure or cholesterol, joint or back pain, type 2 diabetes and sleep apnoea.

How is a Loop-Duodenal Switch (SADI-S / SIPS) Performed?
General Anesthesia
You are placed under general anesthesia for a safe and pain-free surgical experience.
Laparoscopic Access
Several small incisions are made in the abdomen.
A laparoscope and surgical instruments are inserted to perform the surgery minimally invasively.
Sleeve Gastrectomy
The first part of the procedure is identical to a gastric sleeve: a large portion of the stomach is removed, leaving a narrow, tube-shaped stomach.
This restricts food intake and reduces hunger hormones (like ghrelin).
Duodenal Division
The first part of the small intestine (the duodenum, just past the stomach) is divided.
Unlike Roux-en-Y, the pyloric valve (which regulates stomach emptying) is preserved, improving digestion and reducing dumping syndrome.
Intestinal Bypass
A loop of the lower small intestine is brought up and connected to the newly divided duodenum.
This bypasses a large portion of the small intestine, reducing calorie and nutrient absorption.
Leak Test and Closure
The connection (anastomosis) is tested for leaks, and the incisions are closed with sutures or staples.

Recovery, Aftercare & Follow Up
Initial Recovery
You will usually spend 1 night in hospital. Dr Puhalla will visit you the next day for a check up and to discharge you home. You are required to have someone pick you up from the hospital.
Pain Management
Pain medication will be prescribed; rest and limit daily activities is recommended for 1 – 2 weeks.
Hydration
Stay well hydrated, aiming to drink 1.5 to 2L a day taking frequent small sips of water.
Dietary Updates
A 4 staged approach to eating will be guided by the dietitian. This approach starts at liquids, pureed, soft and finally solid foods. This 4 staged approach will be aligned with your dietician appointments post surgery.
Follow Up Appointments
You will have your post operative review appointment 2 weeks after surgery. During the first 6 weeks you will see the dietitian every 2 weeks to guide your dietary upgrades. Regular follow ups with Dr Puhalla at 3 months, 6 months and 12 months to monitor progress and surgical outcome.

After loop duodenal switch surgery it is strongly advised to cease smoking or vaping. This is due to a significant increase to your risk of developing peptic ulcers (stomach ulcers). Smoking or vaping can also delay your healing and recovery process. Ceasing to smoke or vape will also improve your overall health.

How long does a Loop-Duodenal Switch (SADI-S /SIPS) procedure take?
Provided there are no other additional procedures combined, the procedure usually takes 1.5 – 2 hours.
How much weight can I expect to lose?
Weight loss can vary between individuals depending on multiple factors, therefore an exact figure cannot be guaranteed. Most studies show that loop duodenal switch surgery leads to 85-95% of excess weight loss, with most of this occurring in the first 2 years after surgery.
How do I know this procedure is right for me?
To determine your suitability for this procedure, you will need to have a discussion with Dr Puhalla by booking in an initial consultation. The Loop-Duodenal Switch should be my preferred surgical option if I:
- Have metabolic conditions such as diabetes, high blood pressure or high cholesterol
- Eat a diet high in fat, as negative feedback caused by the surgery will help deter you
- Have a poor weight loss history

Is this procedure safe?
The loop duodenal switch operation has a low complication rate, however every surgery carries risks.
Short term risks can include:
- Bleeding

- Blood Clots

Long terms risks can include:
- Reflux
- Malnutrition
- Hernia
- Vomiting
- Ulcers
- Increased frequency of bowel motions

Are there any pre-operative tests prior to surgery?
Yes, in most cases Dr Puhalla will refer you for a pre- operative blood test. He may also refer you for a psychological pre operative assessment to determine surgical readiness.
If clinically indicated, Dr Puhalla may request for you to have a gastroscopy. This is a procedure performed under sedation, where a thin flexible camera is placed down your oesophagus and into your stomach. It can be useful to determine if there is a hiatus hernia, reflux, stomach infections, or ulcers prior to your surgery. A gastroscopy is usually recommended for patients who are over the age of 40, or those who experience reflux symptoms.
Do I need to take multivitamins after Loop-Duodenal Surgery?
This is the most powerful weight loss procedure, therefore it is essential for multivitamins to be taken daily to prevent long-term deficiencies. The dietician may suggest you take other supplements also to ensure you stay well, such as Vitamin D, calcium, zinc etc. This will be determined from your regular blood test results.

Can I smoke or vape after having a Loop Duodenal Switch (SADI-S / SIPS) procedure?
No, smoking or vaping should be stopped prior to undergoing your procedure. Smoking or vaping can have adverse effects and this can delay and impact your healing post operatively. There is also an increased risk of developing peptic ulcers (stomach ulcers) from smoking or vaping. Ca can choose to continue to smoke or vape however we encourage all patients to cease to improve your overall health.
Can I get pregnant after having my procedure?
It is known that weight loss surgery may improve fertility in both females and males. Pregnancy should be avoided in the first 18 months post operatively, due to potential risks to both mother and developing baby. To avoid pregnancy during this time, it is recommended to consider contraceptive options by speaking to your GP. The Oral Contraceptive Pill is less effective after a weight loss surgery, therefore other methods should be used. This is a key consideration if wanting to undergo surgery. If pregnancy occurs earlier than recommended, please contact our team.
Can I have plastic surgery after weight loss surgery?
Yes you can. Some patients may experience loose skin after losing a substantial amount of weight and the result of this is loose skin, which can affect you physically and emotionally. Speak with our team or you GP to arrange a referral for body-contouring surgery by a plastic surgeon.
I have heard gallstones are common after surgery, will this affect me?
