Gallbladder & Bile Duct Surgery
The gallbladder is a small pear shaped organ attached to the liver. Bile produced by the liver is stored in the gallbladder between meals before being released into the small intestines during a meal. The most common problem is forming stones which can cause inflammation or block the bile flow. Key hole or robotic surgery is used to remove the gallbladder with the stones inside.
When does your gallbladder need to be removed?
Gallstones arise from imbalances, often related to a diet high in rich foods. Elevated cholesterol in the bile can crystallize and develop into stones of varying sizes and numbers, from several marble-sized to hundreds of gravel-sized. An excess of bilirubin, a byproduct of hemoglobin, can also lead to stone formation. Stones may be composed of cholesterol, bilirubin, or a mixture of both. Typically, pain is experienced after eating, when the stones shift within the gallbladder.
Gallbladder cancer is typically discovered incidentally, with approximately 0.43% of cases found during laparoscopic cholecystectomy (gallbladder removal). It originates from the innermost layer of the gallbladder wall and is classified as an adenocarcinoma.
During laparoscopic gallbladder removal, bile duct stones are discovered in up to 10% of cases. These stones can obstruct the bile ducts or pancreas, leading to severe pain, infection, or pancreatitis.
Cholangiocarcinoma, or bile duct cancer, can develop in the bile ducts either outside the liver (extrahepatic) or within the liver (intrahepatic). Intrahepatic cholangiocarcinomas manifest as primary liver tumors. In contrast, extrahepatic cholangiocarcinomas typically present with jaundice or biliary duct infections.
How is a gallbladder removal performed?
General Anaesthesia
You are placed under general anesthesia so you’re asleep and do not feel any pain during the operation.
Laparoscopic Access
Several small incisions are made in your abdomen.
A laparoscope (a camera) and surgical tools are inserted to perform the procedure with minimal invasiveness.

Before the gallbladder is removed and Xray with dye is taken from the bile duct in the operating theatre to check for stones.
If a stone is in the bile duct
If the Xray shows a stone different techniques can be used to retrieve the stone ( e.g. retrieve with a small basket, flushing through). Sometimes stones can not be removed during surgery and an endoscopy to retrieve the stone is necessary in separate a procedure (ERCP).

Recovery, Aftercare & Follow Up
Initial Recovery
You will usually spend 1 night in hospital. Dr Puhalla will visit you the next day for a check up and to discharge you home. You are required to have someone pick you up from the hospital.
Pain Management
Pain medication will be prescribed; rest and limit daily activities is recommended for 1 – 2 weeks.
Wound Care
Keep incision sites clean and dry. An incision care guide will be provided to all patients Monitor for any signs of infections or unusual changes. Call the clinic for any concerns.
Diet Change
It is recommended to consume a low fat diet for several weeks after gallbladder removal, to reduce the risk of side effects including indigestion and diarrhoea.
Increasing Exercise Gradually
Lifting and exercise restrictions will be applied for up to 4 weeks post surgery. Do not lift more than 5kgs.

How long does a gallbladder removal procedure take?
Provided there are no other additional procedures combined, the procedure usually takes 1 hour.
How long will I stay in hospital?
You will usually stay overnight in the hospital. In some cases for uninsured patients the procedure will be a day surgery.
