Have you ever wondered what makes someone the right fit for weight loss surgery? It’s not just about BMI; it’s a combination of physical, medical, emotional and lifestyle factors that determine whether a patient will succeed post-surgery. The truth is, being prepared for weight loss surgery involves more than just showing up—it is about commitment and readiness to embrace life-altering changes.

Our surgery lead, Dr Harald Puhalla, understands this, which is why he carefully and holistically evaluates each case to ensure every patient experiences the best possible outcomes.

What Makes a Patient Suitable for Weight Loss Surgery?

Surgeons follow specific guidelines when evaluating candidates for weight loss surgery.

  • The primary factor is a patient’s Body Mass Index (BMI), but other health indicators also play a significant role.
  • Ideal candidates typically have a BMI of 35 or higher or a BMI of 30 or higher combined with comorbidities such as:
  • Type 2 diabetes
  • Hypertension
  • Sleep apnea.

These conditions can be drastically improved or resolved after surgery, making weight loss procedures an attractive option.

Furthermore, a thorough assessment of the patient’s medical history, including any of the following pre-existing conditions:

  • Heart disease
  • Gastrointestinal disorders
  • Hypertension
  • High cholesterol
  • Endocrine disorders
  • Respiratory conditions
  • Liver disease
  • Blood clotting disorders
  • Previous abdominal surgeries
  • Alcohol use disorder (AUD)
  • Depression
  • Anxiety
  • Eating disorders

Surgeons consider a patient’s psychological readiness because your emotional health is an important key success factor in supporting the lifestyle changes post-surgery.

Factors That May Make a Patient Unsuitable for Surgery

Several factors can make a patient ineligible for weight loss surgery.

One of the key concerns is untreated mental health disorders. Conditions like active eating disorders, major depression or addiction issues may make it difficult for patients to commit to the lifestyle changes required for postoperative success.

Additionally, patients who are not willing or able to commit to long-term follow-up care and behavioural changes are considered unsuitable candidates. Surgery is only part of the process. Patients must be prepared for a lifelong commitment to maintaining a healthy diet and regular exercise regimen.

In some cases, patients with chronic conditions such as severe heart disease, uncontrolled diabetes, or active cancer may not be eligible for surgery due to the increased risks associated with the procedure.

Changeable Factors That Can Improve Candidacy

Certain factors that initially disqualify a patient from surgery can be improved with time and effort. For example:

  • Patients who are smokers or excessively use alcohol may need to quit for several months before surgery to reduce the risk of complications.
  • Individuals with poorly controlled diabetes or high blood pressure can work with their healthcare provider to get these conditions under control, making them more suitable for surgery.

Dr Puhalla encourages patients to engage in preoperative weight loss and lifestyle changes. Losing even a small amount of weight before surgery can significantly lower the chances of complications.

Expected Outcomes

Patients who meet the criteria for weight loss surgery can expect substantial improvements in their health. The most significant benefit is, of course, weight loss, with patients typically losing more than 60-70% of their excess weight within the first year after surgery. Beyond this, patients may experience improved or resolved comorbidities such as type 2 diabetes, hypertension and sleep apnea.

Plus, the enhanced mobility, increased energy levels and improved self-esteem that often follow weight loss procedures can drastically improve a patient’s quality of life. However, long-term success requires dedication to maintaining these lifestyle changes.
